Welcome to the Survivor Assistance™ Funeral Concierge Program (“Survivor Assistance”), an added benefit available to eligible individuals, their beneficiaries, and family members. This service is provided in partnership with DocuGuardian and Titan Concierge (“Service Partners”) to offer nationwide support with funeral planning and coordination.

This Survivor Assistance™ Terms of Service (this “Agreement“) is a binding contract between you (“Member,” “you,” or “your“) and Survivor Assistance™. This Agreement governs your access to and use of the Services.

THIS DOCUMENT CONTAINS VERY IMPORTANT INFORMATION REGARDING YOUR RIGHTS AND OBLIGATIONS, AS WELL AS CONDITIONS, LIMITATIONS, AND EXCLUSIONS THAT MIGHT APPLY TO YOU. PLEASE READ IT CAREFULLY.

THIS AGREEMENT REQUIRES THE USE OF ARBITRATION ON AN INDIVIDUAL BASIS TO RESOLVE DISPUTES, RATHER THAN JURY TRIALS OR CLASS ACTIONS.

THIS AGREEMENT TAKES EFFECT WHEN YOU SCROLL THROUGH THE TERMS OF THIS AGREEMENT AND BY ACCESSING OR USING THE SERVICES (the “Effective Date“). 

BY ACCESSING OR USING THE SERVICES YOU: (A) ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTAND THIS AGREEMENT; (B) REPRESENT AND WARRANT THAT YOU ARE EIGHTEEN YEARS OF AGE OR OLDER; (C) REPRESENT AND WARRANT THAT YOU ARE NOT PROHIBITED FROM ACCESSING OR USING THIS WEBSITE OR ANY OF THIS WEBSITE’S CONTENTS, GOODS OR SERVICES BY APPLICABLE LAW; AND (D) ACCEPT THIS AGREEMENT AND AGREE THAT YOU ARE LEGALLY BOUND BY ITS TERMS.

IF YOU DO NOT AGREE TO OR ACCEPT THESE TERMS, YOU MAY NOT ACCESS OR USE THE SERVICES.

  1. Acceptance of Terms

By using the Survivor Assistance™ Funeral Concierge Program, you agree to these Terms & Conditions (“Terms”). If you do not agree, please do not use the services provided.

  1. Program Description

Survivor Assistance™ provides access to funeral concierge services, including guidance, planning support, and coordination through Service Partners and is delivered  through Program Sponsors. Program Sponsors can include but are not limited to: employer groups, unions, associations, financial institutions, insurance companies, government entities, including governmental agencies, community of interest networks, clubs and religious organizations.  The program does not include financial products or funeral service payments.

  1. Eligibility

Survivor Assistance™ is available to eligible individuals as defined by the Program Sponsor. 

  1. Service Partners

Services are facilitated by DocuGuardian and Titan Concierge. By engaging with these services, you agree to their respective terms, conditions and policies, which are independent from Survivor Assistance™ and any Program Sponsor.

  1. Limitations of Use
  • Services are intended for lawful and personal use only.
  • The program may not be used for commercial gain, reselling, or non-family coordination.
  • Survivor Assistance™ reserves the right to suspend or terminate access to the program for misuse.

Survivor Assistance™ does not provide legal, financial, or medical advice. All services are informational in nature and should not replace professional consultations.

  1. Limitation of Liability

To the fullest extent permitted by law, Survivor Assistance, the Program Sponsor and its partners disclaim all liability for any damages resulting from your use of or inability to use the services. Use of the program is at your own risk.

  1. Modifications

Survivor Assistance™ reserves the right to update or modify these Terms at any time. Continued use of the service constitutes acceptance of the revised Terms.
